Children's Community Nursing
What does the service do?
Our teams provide support and advice for children, young people, and their families with both acute and chronic medical and surgical conditions and complex health needs. We undertake a wide variety of nursing procedures and direct nursing care within the community setting.
We will act as the child's and family's advocate and can attend meetings with the family to offer support and liaise with other service providers.

Age Range
The service is available for young people age 0-18 and up to 19 if accessing full time education.

We are only able to accept referrals from a professional either the hospital treating your child/young person or your GP.
Other professional involved in the care of your child can also refer if required.
