The Adult Social Care Hub provides guidance on preventive and community care services within Nottingham City. If you feel further support is needed, you can submit an online referral directly to Adult Social Care.
If you have already had an assessment and want to understand potential care costs, you can complete an online financial assessment to receive an estimated contribution.
Referrals are reviewed by Adult Social Care and passed to the most appropriate team, who will contact you if needed.
