Who can get a short break?
Short breaks are for those children and young people who may experience difficulties accessing universal services.
We offer short breaks to children and young people who:
- are from age 5 to 18 years old
- live in Nottinghamshire County
- score 5 points or more on the assessment application.
How your application is scored
There are some key criteria on your assessment application that receive points. Your child needs 5 points or more to receive a Nottinghamshire County Council funded short break.
The main criteria and their points are:
- Age of the child (if the child is aged 12 to 17 years) = 2 points
- Middle or higher bands Care Disability Living Allowance (DLA) = 3 points
- Higher level mobility Disability Living Allowance (DLA) = 1 point
- Higher disability living part of Personal Independence Payment (PIP) = 3 points
- Higher mobility part of Personal Independence Payment (PIP) = 1 point
- Education Health and Care (EHC) plan = 2 points
- Education High Level Needs (HLN) funding = 2 points
Please note: The education High Levels Needs (HLN) funding is paid to schools directly from Nottinghamshire County Council. The HLN is meant to support the child or young person to reach good outcomes in school. If you are unsure as to whether your child is receiving HLN funding, please speak to your child’s school.
If you are unsure about the criteria that applies to your child, you can email the Short breaks Assessment and Review team for support at sbassessment.review@nottscc.gov.uk
